Ally Accessibility Report Reports a File Not Linked to Course

Description:
A file which is not in the course or linked within the course can appear in the course accessibility report.
Steps to Repeat:
1. Log in as a system administrator
2. Go to System Admin > Courses
3. Search for and enter an affected course
4. Go to Control Panel > Course Tools > Accessibility Report
5. Locate issue "The document contains images without a description"

OBSERVED RESULT:
Report shows a file which does not appear to exist in course.

EXPECTED RESULT:
Report only shows course files.
